Breckpoint, Inc., a third-party administrator (TPA), significantly enhances the customer service experience and employer web-based capability.
This website highlights the Freedom ICON I Plan, coverage conditions, and provisions. The benefit plans feature our same philosophy of affordable cost and easy-to-understand coverage provisions.
There are two “Freedom ICON I Plans” that cover primary and emergency care services and generic prescriptions.
To review these plans, click here or look for “Freedom ICON I Plan” on the menu bar at the upper left, and select the plan you wish to review and download brochures for all our plan offerings if desired.
To get a personalized quote for the Freedom ICON I plans for your company, click here or on “Get a Quote” in the menu bar at the upper left. Fill in the short questionnaire based on your estimated enrollment and submit. A personalized estimated quote will be emailed to you instantly.
With our new product offerings and no-age-band pricing, employers must complete two documents and submit them. Both documents can be accessed on this website or call us at 888-703-7667 and we will personally answer your questions and assist you with the enrollment process.

Upon submitting these forms, you will receive an enrollment link with a menu of benefit options for your Employees to select the coverage that is appropriate for their families and needs. Employers can expect a brief onboarding session with Breckpoint Customer Service Specialists on the use of the Employer Web Portal.

Employer Enrollment Packet with DocuSign - Step 2 – Breckpoint will set up your enrollment portal account and email logins to you
- Step 3 – Fill out the Employee Eligibility Census
- Step 4 – Login to your Employer portal at Breckpoint and securely upload your completed Census file
